Fill in the 8D report online
Start with the header: problem, customer, product, complaint number, opening date and the person who leads the 8D. The disciplines follow. For each one you see the guiding questions a customer or auditor will typically ask. You do not need to finish in one sitting: your entries stay in your browser until you reset them.

Header
D0Preparation and emergency response(optional)
Is 8D the right method? Which emergency response protects the customer in the first hours?
D1Team
Who is on the team (name, function, role)? Who leads, who sponsors?
D2Problem description
What, where, when, how many, how detected? What is affected – and what similar is not (is / is not)?
D3Containment actions
Which stock is blocked and checked? How are checked parts marked? From when is the customer protected?
D4Root cause analysis
Why did the defect occur, and why was it not detected? How is the cause verified (5 Why, Ishikawa, trial)?
D5Choose corrective actions
Which action removes which cause? How was it shown before implementation that it works?
D6Implement and validate
Who implements what by when? Which data prove effectiveness in series production? When does containment end?
D7Prevent recurrence
Which documents, FMEA, control plans and trainings are updated? Where are similar products or processes?
D8Closure and recognition
Does the customer agree to closure? What did the team learn, and how is it recognized?

What belongs in each discipline
| Discipline | Core content | Typical mistake |
|---|---|---|
| D0 Preparation | Emergency response, decision to run an 8D | Skipped although the customer needs protection at once |
| D1 Team | Names, functions, lead, sponsor | Quality department only, production missing |
| D2 Problem | 5W2H, is / is not, quantities, photos | A suspected cause instead of a description |
| D3 Containment | Blocking, sorting, marking, effective from when | No evidence that containment works |
| D4 Root causes | Occurrence and non-detection cause, verification | Only one cause, not verified |
| D5 Actions | One action per cause, effectiveness shown upfront | Training as the only action |
| D6 Implementation | Dates, owners, data from series production | Closed without effectiveness data |
| D7 Prevention | FMEA, control plan, documents, similar products | Only the one part gets fixed |
| D8 Closure | Customer agreement, lessons learned, recognition | The team is never told |

Five tips for an 8D report your customer accepts
- 1
Know the deadlines
Many customers expect D3 within 24 to 48 hours, D4 after about ten working days and closure after roughly 60 days. Agree the deadlines at the start and note them in the header.
- 2
Name two causes
Customers often reject an 8D report because only the occurrence cause is given. Always describe why the defect was not detected as well.
- 3
Verify the cause
A cause is only confirmed when you can switch the defect on with it and off again with the action, for example in a trial.
- 4
Close with data
In D6, prove effectiveness with figures from running production, for example zero defects in three weeks of series production with 100 percent inspection.
- 5
Think system-wide
In D7, ask where the same defect could occur elsewhere and update the FMEA, control plans and work instructions.
Before you send the report to the customer, run through the 8D report checklist. It lists the points customers and auditors use to judge an 8D report.
From a single 8D to a controlled CAPA process
A template helps with a single case. Once several complaints run in parallel, you need deadlines, owners and evidence in one place. In docxpert the nine phases of CAPA management cover the content of an 8D report – from team and problem description through containment and root cause to the effectiveness check. Changed documents and trainings are linked to the action.
Frequently asked questions
Is the 8D report template really free?
Yes. You can fill in the template online without signing up, save it as a PDF and print the blank version. There is no cost and no account is created.
Does docxpert store my complaint data?
No. Your entries stay in your browser's storage. For the PDF export they are sent once to our server, which creates the PDF and does not store the data.
Is the template available as a Word or Excel file?
The template is available as an online form and as a PDF. Many customers prescribe their own form; in that case copy the content from the PDF into their form.
Do I need D0?
D0 is optional. It makes sense when the customer must be protected before the team is in place, or when you first decide whether a full 8D is needed.
How long does an 8D report take?
It depends on the problem. Typical is containment within one to two days, a verified root cause after about two weeks and closure after six to eight weeks, once effectiveness data from series production is available.
