## When to use the checklist

The most common reasons customers reject an 8D report are a vague problem description, an unverified root cause, a missing non-detection cause and closure without evidence of effectiveness. This checklist covers exactly those points.

Use it in two places: as a self-check before your team releases the report, and as an assessment grid when you request an 8D report from a supplier. To fill in the report itself, use the free [8D report template](https://docxpert.io/en/tools/8d-report-template); the [8D report](https://docxpert.io/en/guides/8d-report) guide explains the method.

## Checkpoints

### D1 team and D2 problem description

- The team is cross-functional (at least quality and production) and has a named lead.
- A sponsor with decision-making authority is named.
- The problem is described from the customer's point of view, not as a suspected cause.
- What, where, when, how many and how detected are backed by figures (5W2H).
- An is / is-not analysis shows what is affected and what similar is not.

### D3 containment

- All potentially affected stock is named: at the customer, in transit, in the warehouse, in production.
- Blocked stock has been inspected or sorted, and the result is documented.
- Inspected parts are clearly marked, and the customer knows the marking.
- The point from which the customer is protected (clean point) is stated.

### D4 root cause analysis

- The occurrence root cause is named.
- The non-detection root cause is named.
- A suitable method is visible (5 Why, Ishikawa, fault tree, data analysis).
- The cause is verified, for example by a trial that reproduces the defect.
- The cause chain ends at a system cause you can influence, not at "human error".

### D5 and D6 actions and implementation

- Every root cause has at least one corrective action.
- The actions address the cause and go beyond training or reminders.
- Owners and dates are set.
- Effectiveness is proven with data from series production (period, quantity, inspection scope).
- The end of containment is justified and comes only after effectiveness is proven.

### D7 prevention

- FMEA, control plan and work instructions are reviewed and updated where needed.
- Similar products, processes and sites are checked (read across).
- Affected staff are trained, and the training is documented.
- Findings feed into lessons learned or the development of new products.

### D8 closure and form

- The customer has agreed to closure, or agreement has been requested.
- All deadlines set by the customer are met, or deviations are explained.
- The report is clear, complete and free of internal abbreviations.
- Evidence (photos, measurements, document versions) is attached or referenced.

## How to assess the result

Release an 8D report only when every point in D3 and D4 is met. Gaps in D7 and D8 are acceptable in an interim report as long as they are named with a date. For supplier 8D reports, document your assessment and feed it into supplier evaluation.

Recurring gaps, such as a non-detection cause that is always missing, point to a training need in the team or at the supplier. The [root cause analysis](https://docxpert.io/en/guides/root-cause-analysis) guide and the [CAPA checklist](https://docxpert.io/en/checklists/capa) help here.
